Training Guidelines for Microsclerotherapy
The BAS recommends; Those wishing to train in this field should look for courses which will cover the following.
Appropriate Practitioners
Doctors Dentists Nurses* Others* *Under medical Supervision
Appropriate Trainers Must be Venous Specialists or have at least 5 years experience which can be evidenced.
Training
Theory Theory with reference to the evidence base where appropriate, Supported with written material. Written assessment for students to demonstrate understanding and achievement of learning outcomes.
Practical Demonstration Supervised practice for students to achieve and be observed to demonstrate competencies.
Curriculum
1. Anatomy and Physiology Learning Outcomes • Students will understand how deep and superficial venous system in the legs works. • Will be able to explain the normal function of the leg veins to the patient. • Will understand the mechanisms of reflux, ambulatory venous hypertension and chronic Venous Insufficiency • Will understand the classification of veins and be able to apply this when selecting patients suitable for treatment. • Will be able to relate knowledge of anatomy and physiology/pathophysiology to inform patient assessment and selection. • Be able to answer questions related to physiology, pathophysiology and aetiology frequently asked by patients.
2. Pharmacology Learning Outcomes • Will understand the mechanisms of action of sclerosing agents, including detergents, osmotics and chemical irritants. • Will be able to name each of the common brands and which group it belongs to. • Will be able to explain the legal issues relating to drugs, including; • Supply,prescription and administration of licensed and unlicensed drugs. Storage, disposal and record keeping. • Will be able to describe the risks and complications associated with each of the drugs.
3. Equipment and Environment Learning Outcomes • Will be able to list the equipment required for safe practice • Will be able to list consumables required • Will be able to describe the safe environment for practice • Will be able to refer to the relevant Health and Safety laws and regulations that apply.
4. Patient Assessment Learning Outcomes • Will be able to list exclusion and inclusion criteria • Will be able to conduct a medical history • Will be able to conduct a physical assessment of the patient and recognise CEAP2 veins and symptoms of CVI. • Will be able to identify patients not suitable for treatment and recommend referral. • You will have an overview of treatment options for CEAP2 Veins
5.Consultation Learning Outcomes • Will be able to explain the treatment process • Possible risks and complications • Manage expectations • Consent the patient according to GMC guidelines • Explain post treatment instructions • Provide written information • Answer frequently asked questions appropriately. • You will have an understanding of alternative treatment options
6.Injection Technique Learning Outcomes • Will be able to explain the principals of a safe technique • Be able to inject safely achieving blanch • Be able to recognise when the solution is extravascular and take appropriate steps • Demonstrate comfortable and dignified positioning of the patient • Demonstrate safe and comfortable positioning of yourself • Identify veins suitable for treatment • Demonstrate competency injecting both telangiectasia and reticular veins • Be able to identify veins requiring additional compression and apply. • Maintain a safe environment • Demonstrate safe handling and disposal of sharps • Demonstrate aseptic technique
7. Post Treatment Compression Learning Outcomes • Be able to discuss the evidence base supporting the use of compression. • Identify contraindications • Discuss the different kinds of compression • Demonstrate an understanding of graduated compression hosiery and the different classes. • Be able to measure and fit appropriate compression
8. Results and Complications Learning Outcomes • Describe how you would manage expectations • Explain how you can minimise risk of complaints and complications. • Identify the different complications and explain them • Explain how you would manage each of the complications
9. Medico legal Aspects Learning Outcomes • You will have an understanding of the professional and legal responsibilities you bear in your practice • You will have sufficient knowledge of the information and bodies concerned to access information resources available
10. Documentation and Record keeping Learning Outcomes • Be able to list the information that should be documented and why.
